Required Forms for Coaches & Helpers

Application for head/assistant coach, camps, leagues

Every season, AVC accepts applications for coaches for our JO traveling teams, non-traveling developmental league, camps or other events. Initially, please complete the application and the Covid-19 waiver in the first section below. The Coach Coordinator or Board will contact the applicants over the next few weeks as we prepare for our upcoming traveling season and provide further direction.

1. AVC Coaching Application & Covid-19 Waiver

  1. AVC needs to have an application on file for each coach for each season. Click HERE to complete the application for the 2020-2021 season.
  2. NEW THIS YEAR:  A COVID-19 Preparedness Plan Waiver is also required to be on file for each coach for the 2020-2021 season. Click HERE to complete the waiver form. 

2. JVA Background Screening/APS Training Process

AVC is a member of Junior Volleyball Association (JVA). To be eligible to coach or assist, JVA requires both a background check and abuse prevention training to be completed prior to the start of the season. Click HERE to access the background screening and training process.


  • All first time coaches and helpers are required to complete this process before the start of the season. 
  • For all returning coaches and helpers, this needs to be completed every other year.
    • If you don't know if you are due to complete a new background check, please inquire with the Coach Coordinator or AVC.
  • You must sign in to your SportsEngine account. JVA uses SportsEngine for some of their website processing. If you don't have one, please set one up prior to starting the background check/training. Click HERE to create a new SportsEngine account.
  • Be sure to select Alexandria Volleyball Club (MN) when prompted to select a club.
  • The cost is $38 and will be reimbursed to you via the Alex Volleyball Club. Please submit payment yourself and then send  the receipt of payment to

3. W-9 Tax form

Annually, AVC reports wages to the IRS so every coach needs to have a W-9 form on file with AVC before a coach receives a paycheck. Print off the W-9 form (see link below), complete/sign it and return via email to or mail it to AVC, PO Box 93, Alexandria, MN 56308

4. Time Sheets

AVC  coaches and assistants for camps and events need to log his/her hours on a time sheet and submit the final signed sheet to AVC via email or mail. If you need to use more than one form, please submit them to AVC together. The email address and mailing address is noted on the bottom of the time sheet. Payment will be mailed to you after a completed and signed time sheet is received, as long as AVC has a W-9 on file.
